CMARC - Care Management for At Risk Children

Goals 

The goal of Early Childhood Care Management is to improve your child's care by linking you to services that will meet the specific needs of your child and family.

Eligibility 

The program is available to children from birth to age 5 years who:

  • have long term medical conditions or
  • are dealing with challenging levels of stress

Payments

The program is provided at no charge to the family regardless of insurance status.

Referrals

Referrals for the program may be received from the child's doctor, a hospital, the Department of Social Services (DSS), Woman Infants and Children (WIC), an insurance company, or a community member. Families may also complete a self-referral.

CMARC Referral Form

When the care manager receives a referral, they will talk with the child's parent to determine if the child and family could benefit from CMARC services.
