Primary Care for children from birth to age 21

Grade Level Entry Vaccine Requirements

 Kindergarten

*At all ages and grades, the number of doses required may vary by a child's age and when they were vaccinated. 

DTaP 5 doses 
Polio 4 doses
Hib (4-YEAR OLDS ONLY) 3-4 doses
MMR (or 2 measles, 2 mumps, 1 rubella) 2 doses
Hepatitis B  3 doses  
Varicella 2 doses
Pneumococcal conjugate (4-YEAR OLDS ONLY)  4 doses


Services

The Union County Public Health Child Health Clinic provides primary care to children from birth to 21 years. Well child physical exams, immunizations, management of chronic conditions, and sick visit services are provided.

Eligibility

Patients must be residents of Union County between the ages of 0-21 years.

Payments

Medicaid and private insurance are accepted. Uninsured patients are welcome.

Fees for uninsured patients will be determined by a sliding fee schedule based on family income.

Vaccines are available free of charge to uninsured children.

Appointments

Child Health appointments are available Monday through Friday.

Notice Regarding New Patients

Due to capacity limits, we are currently only accepting new patients who:

  • Are uninsured  OR
  • Have a sibling who is already an established patient in our Child Health Clinic


Services include:

Preventive health services, immunizations and physical exams are provided on a regular schedule of visits based on the guidelines of the American Academy of Pediatrics.

Routine Physical Health Exams include:

  • Height/Weight
  • Vision Screening
  • Hearing Screening
  • Developmental Screening
  • Head to Toe Physical Exam by medical provider
  • Age appropriate laboratory tests
  • Immunizations
  • Nutrition Counseling

Chronic conditions such as asthma, allergies and eczema can also be managed along with referrals to specialists when appropriate. A social worker is available in clinic to see patients who need assistance with obtaining resources.
