Vital Records

Visit our Vital Records page for home birth registration, affidavit of parentage, and medical record requests.

If you have questions regarding the status of a pending birth or death certificate, call 704-296-4853 or email Vital Records directly.

If you need to obtain a copy of a birth or death certificate, please call the Union County Register of Deeds at 704-283-3843 or visit Birth, Death & Marriage Records.


Pandemic InfographicCommunicable Disease Prevention

North Carolina law requires medical providers to report cases of over 80 different communicable diseases to the local health department. The Communicable Disease Team monitors these reports for possible outbreaks. When outbreaks are identified, the team investigates the source of the infection and how to resolve the outbreak to minimize the number of residents affected.

Child Care Health Consultants provide expert information, resources, and referrals to local child care facilities to help them maintain safe environments that will support healthy growth and development.

Early Childhood Care Managers work with children from birth - 5 years who have chronic medical conditions, special needs, or are dealing with challenging levels of stress.  They help link families with the services and resources they need to provide appropriate care for their child and to meet the special needs of the child and family.
